Well-Child Visits

Regular checkups are important for your child’s physical, emotional, and social health and development. Well-child visits help prevent health problems, track growth and milestones, and give you an opportunity to discuss any concerns. These visits are available for children from birth through age 21.

At a well-child visit, your primary care provider will:

  • Perform a physical exam.
  • Administer any necessary immunizations.
  • Track your child’s growth and ask about development and behavior.
  • Visit with you about illness prevention, nutrition, and physical fitness.

Adult Preventative Visits

An adult preventive visit is an annual appointment designed to help prevent illness and identify health concerns early, often before symptoms appear. During the visit, you’ll review your overall health and well-being and have the opportunity to discuss any concerns with your primary care provider.

Medicare Annual Wellness Visits

As part of your Medicare coverage, you are entitled to a free annual wellness visit once per year. This visit gives you the opportunity to work with your primary care provider to develop a personalized prevention plan and take full advantage of preventive care benefits covered at 100%. It provides a snapshot of your current health as a baseline for future visits and is designed to catch potential health issues early.

CCCHC offers annual wellness visits to Medicare patients in-clinic, virtually, or in-home.
