Virtual Clinic
CCCHC offers virtual care services Monday - Friday, 8:00 a.m. - 5:00 p.m. You will need MyChart access, strong internet connection and a computer, tablet or cell phone device with camera and audio. Schedule an appointment today through MyChart or by calling the clinic.
TytoCare
CCCHC uses the TytoCare device to enhance virtual medical exams. TytoCare allows your provider to receive high-quality digital sounds of your heart and lungs, as well as clear images and video of your ears, throat, and skin. The device can also measure heart rate and body temperature, helping your provider deliver thorough care during a virtual visit.
Remote Patient Monitoring
Remote patient monitoring is a form of virtual care that allows a CCCHC provider to monitor and manage their patients’ chronic conditions. It can also prevent health complications in patients who aren’t able to easily travel.
There are many symptoms and conditions that can be tracked through remote patient monitoring, including:

- Diabetes
- Weight loss or gain
- Heart conditions
- Chronic obstructive pulmonary disease
