Throughout July 2026, Coal Country Community Health Center will be holding preventive wellness exams and/or sports physical for children and adolescents needing sports physicals for the 2026-2027 school year.
When calling your preferred clinic to schedule your youth wellness exam, please indicate if you want a sports physical only. Most insurances cover a wellness/preventive exam that includes a sports physical. If you want a sports physical only, the cost is $40 at the time of the visit. Parents/guardians should check insurance eligibility prior to scheduling.
This is also a great opportunity to update immunizations! A parent/guardian must be present to consent to immunizations.
IMPACT testing is available for Beulah students entering grades 7, 9, and 11 at the Beulah Clinic on select dates. Please request when scheduling your wellness exam.
Students who complete their exam in July will be entered for a chance to win a door prize!
Important Reminder: Sports physical and immunization forms must be completed and signed prior to the appointment and brought to the exam.

Local healthcare organizations seek community input
CCCHC has teamed up with the UND Center for Rural Health, Custer Health, Hilltop Home of Comfort, Knife River Care Center and Sakakawea Medical Center to gather information about the communities we serve and learn more about how local services are used. The results of this survey will be used to develop a plan to improve and expand the care we provide.

CCCHC Expands Care in Beulah Schools
Coal Country Community Health Center, in partnership with the Beulah School District, is excited to offer expanded services of the IMPACT (Integrating Mental Health, Physical Health, and Continuity of Care Together) Program to students. On January 19, 2022, CCCHC opened a new school-based site located in the Beulah High School. Kayla Ternes, PA-C provides primary care services to students in PreK through 12th grade. Appointments are available Mondays, Wednesdays, and Fridays from 8:00 a.m. - 12:00 p.m.

Youth Wellness Exams Offered at Beulah & Hazen School Clinics
Coal Country Community Health Center will be holding preventative wellness exams for students in PreK-12th grade in May. A wellness exam may include a sports physical for the 2023-2024 school year if requested. The sports physical will be valid through the 2024 school year.
