Coal Country Community Health Center will be holding preventative wellness exams for students in PreK-12th grade in May. A wellness exam may include a sports physical for the 2023-2024 school year if requested. The sports physical will be valid through the 2024 school year.

- You must bring the sports physical form to appointment with the parent/athlete section completed.
- This is a great opportunity to update immunizations! A parent or guardian is encouraged to accompany their child for consent of updating immunizations. The below forms will be needed at the time of appointment.
If you’re on Medicare, you’re entitled to a free annual wellness visit once every year. Earn a $15 gift card if you complete your AWV before Dec 31, 2022!

The Community Builder program is a unique initiative designed to support charitable organizations and nonprofits that RDO team members are passionate about and serve the greater good in their communities. This year’s program focused on supporting youth mental health nonprofits.
