If you’re on Medicare, you’re entitled to a free annual wellness visit once every year. Earn a $15 gift card if you complete your AWV before Dec 31, 2022!
This visit is a chance to work with your CCCHC health care team to develop a personalized prevention plan, and take full advantage of preventive care benefits that are covered at 100 percent. It provides a snapshot of your current health as a baseline for future yearly visits, and is intended as a preventive service to catch potentially serious health issues early. CCCHC offers annual wellness visits to Medicare patients in-clinic, virtually and in-home.
Complete your AWV before December 31, 2022 to earn a gift card to your local grocery store (Krause's SuperValu in Hazen, Bronson's Marketplace in Beulah, Hinrich's SuperValu in Killdeer) or local restaurant (Crossroads in Center). Contact CCCHC to schedule today.
Did you know it's important to bring our kids to the clinic when they are well, too? During the months of August and September, Coal Country Community Health Center will be giving children ages 3-6 a gift when seen for a well-child check. Call to schedule at any of our locations today!
