Call to schedule your sports physical appointment at CCCHC today! Bring the Sports Physical Form with and have the Parent/Athlete section completed. Great opportunity to update immunizations! Parents please accompany your student athlete.

Youth Wellness Exams + Sports Physicals: Extended Hours Available

Beginning in June, Coal Country Community Health Center will be holding preventive wellness exams and/or sports physical for children and adolescents needing sports physicals for the 2023-2023 school year. Extended hours are available! When calling your preferred clinic to schedule your youth wellness exam, please indicate if you want a sports physical only. Most insurances cover a wellness/preventive exam that includes a sports physical. If you want a sports physical only, the cost is $40 at the time of the visit.

CCCHC Foundation Scholarship

Coal Country Community Health Center Foundation knows how important it is that local residents gain the skills to be our community’s health care providers of tomorrow. We are pleased to invite graduating seniors who are seeking a career in a medical field (e.g., physician, nurse, therapist, technologist, pharmacist, etc.) to apply for a CCCHCF scholarship. Four $500 scholarships will be awarded.
