Important Reminders
- CCCHC will collect a $25.00 fee at the time of the appointment. Visits will only pertain to the student’s sports physical.
- You must bring the sports physical form to appointment with the parent/athlete section completed.
- This is a great opportunity to update immunizations! A parent or guardian is encouraged to accompany their child for consent of updating immunizations. The below forms will be needed at the time of appointment.

Beginning in June, Coal Country Community Health Center will be holding preventive wellness exams and/or sports physical for children and adolescents needing sports physicals for the 2023-2023 school year. Extended hours are available! When calling your preferred clinic to schedule your youth wellness exam, please indicate if you want a sports physical only. Most insurances cover a wellness/preventive exam that includes a sports physical. If you want a sports physical only, the cost is $40 at the time of the visit.
