Welcome to Sandhills Community Health Center and Coal Country Community Health Center. We appreciate the opportunity to serve you and your health care needs.
We do ask that you report to the clinic 15 minutes before your scheduled appointment time in order to register you as a new patient. We do have the new patient registration form available on our website for your convenience and you may choose to complete this prior to your appointment and bring it with you. We do also ask that you bring your insurance cards/information with you to your appointment.
As a community health center, we are excited to offer you the Sliding Fee Scale program. This is an income based program, with the income guidelines set by the federal government. If your household income falls within the federal guidelines, you can qualify for a discount off of services rendered here at the clinic and prescription drugs received from Beulah Drug. Please ask us about the Sliding Fee Scale program.
