Everyone is welcome at Coal Country and Sandhills Community Health Centers. We are committed to providing affordable, quality medical care.
We are preferred providers of:
Blue Cross Blue Shield, Medicare, Medicaid, America's PPO, PPO Next, Tricare, Preferred One, Humana, Secure Horizons Direct, Advantra Freedom, Three Rivers Provider Network, BCBS Medicare Blue PPO, Railroad Medicare and Medicare DME
Because Coal Country and Sandhills are Federally Qualified Community Health Center, Medicare will pay for some additional preventative services not normally covered by Medicare.
Most private insurance is accepted at Coal Country and Sandhills.
We offer a Sliding Fee Scale Discount Program (see below) to all patients that qualify.
Co-Payments: Patients with coverage that requires co-payments must pay that amount at the time that care is provided.
Discount Program
Coal Country and Sandhills Community Health Centers are focused on delivering health care to the entire community, our Sliding Fee Scale Discount Program provides a way for lower income individuals and families to receive health care services. It discounts the cost of medical or dental care for those patients with incomes below 200 percent of the Federal Poverty guidelines. The sliding fee scale allows patients to pay according to their own ability and is funded by a federal grant, so no one has to go without health care
You are eligible with or without health coverage.
You can qualify for the discount even if you have health insurance. You are responsible for the co-payment established by your insurance company, the sliding fee scale will be applied to any remaining balance according to your qualifying percentage. All eligible uninsured patients must pay a co-payment of $10 for their care at the time of the office visit and be will be billed for any remaining charges after the discount has been applied. All co-payments are due at the time of your visit and your account cannot be discounted until it is paid.
How do I apply?
You may download our application here and mail with income verification or you may complete the application at the Health Center. Patients may complete an application at anytime, it's all confidential and even patients with Medicare or other forms of health insurance may qualify. No formal appointment is needed. Patients wanting to receive the discount for a specific service date should complete the application no later than one month from that service date. Patients must provide proof of income for the Sliding Fee Scale Discount Program at the time the application is completed.
What information do I need to complete the application?
Each member of the household must be listed on the application and proof of income for each must be provided. The discount will not be applied without proof of income.
How often do I apply?
Applications must be completed once a year or when your income or family size changes significantly.
Does each person in my family need to apply?
One application may be completed for you, your spouse and your children under the age of 18 who are living in the household.
How do I find out the amount of my discount?
CCCHC’s or Sandhills' Financial Consultant will process your application and inform you of your discount.
